I've never seen a change in mileage with the two 6.0 I've put exhaust on. I've never put an aftermarket intake on so I can't say much for that.
Super Economy tune from LIPD would get you the best results.
diesel world article says you can, I posted the info a week or so ago. Milage wasnt much but it was 18+ as I recall, almost 20 with the use of synthetic fluids in a 4x4.
Started out with 17mpg stock, ended up with 23mpg total after the above, a chip and a bed cover!
I think a tune would be your best increase. I have done exhaust mods in the past with no increase. As far an air filter, I would stay with the OEM, it's probably the best filter I've ever used.
I have found the premium pumps mixed in with the regular diesel pumps. No prices out at the road, just the pumps. I've had to look sometimes while traveling.
Unless I'm mistaken, add a good cetane boost, such as Diesel Kleen, when you fuel up and you'll have premium diesel. Diesels love more cetane. Mine runs smoother, starts easier, and gets about 0.8 mpg better.
