PSD "Cough

My 6.0 PSD has developed a "cough" when towing. I have been advised it
might be due to the low sulfur diesel we are currently using. When it
coughs it spits out a puff of black smoke. This "cough" usually occurs
when under load up hill, or trying to accelerate, 2500 RPMs to about
3500RPMs. Most recently the weather was clear, about 85-90 degrees. Occasionally it coughs a small cough when I let off the
throttle. I put an additive, cetane booster, in the tank on a recent fill up and no cough until I filled again, then about half a tank later it began to cough again.
Any ideas or advice? Thanks for any advice or knowledge.
