I am replacing my '61 F-100 windshield and have the old one to give away. It does have one bulls-eye in the middle, but it might be better than what you have now. It is in MD, just south of Gettysburg, PA.

Had glass shop replace windshield in 65 and while they looked I searched few Ford parts supplier's and found LMC, and NPD carry them and can run anywhere from $150 to $250. There are also couple more suppliers on the west coast but the 2 mentioned would probably be cheaper as far as shipping cost is concerned.
I had a new windshield installed in my ´66 F-100 last year. The windshield was made in Finland and I let a glass shop do the installation, worth every penny (around $200).
Had one acouple years ago installed in my 65; (Same Glass as 66 model etc.). Glass came from Nashville Glass company. Had installed for a total of I beleive $225. (Slightly green tint in glass)
Ordering glass from reproduction suppliers is great; but the shipping could cost up to around $100; Check with any reliable auto glass installer in your area and you'll be surprise its still a very acessible glass item to get.
