i was driving on the hyway going about 100 at like 4000 rpms when i started to smell smoke look behind me and tones of white smoke poring out .i thought the moter was blown i took the heads off to find the piston with a hole burnt right down the side all the way through it looks like it got so hot it plasmed some of the piston to the top of the cylinder. it looks like the cylinder will be ok im going to buy all new pistons and rings and hone the cyls in the car .but i dont know what caused the piston to fail and dont want it to happen again . it was the second cyl from the top the to first pistons are so shiny there like new but the rest look slightly dirty i need help .thanks oh its a 91 e150 efi van eith a 302 .

Mine was piston #3, after tearing down the motor, I had found that a pack rat had chewed off my injector wire to #3. I live in the desert and hardly drive my truck, so the pack rats had made a nice home on my intake.
that injector probably went bad, ran lean, and detonated burning a hole in the piston because it didnt have the gas to cool the piston crown
You nailed it right on the head. That's exactly what happened. Time for new injectors with the rebuild. And run at least one tank full of Chevron or Texaco gas with Techron every month to keep the injectors clean.
