Weird Starting Problems...

Number (2) makes me suspect a fuel delivery problem, maybe a weak pump, or maybe a voltage interuption, or drop to it.
The fuel pump power relay, located in the under hood power distribtion box, is a common problem & deserves to be up there on your suspect list.
You might try swappng it with a relay not needed to start, or run the engine, like the A/C relay & see if things get better, if so, replace the fuel pump powr relay.
You can get them for about $5 at most autoparts stores.

Shell is the most recent company to have let high sulphur content fuel get to market under it's & a number of other bands that used Shell based gasoline for resale.
They managed to wipe out a good number of fuel pumps & sending units & have been successfully litigated against for it.
Chevron now has a upgraded product called "Techron Concetrate Plus" that they say will help clean up & prevent the corrosion, caused by high sulphur content fuel, on fuel pumps & sending units.
So if the relay swap doesn't work & you think maybe fuel pump sulphur corrosion is a possibility, you might consider giving the Techron a try, at the perscribed dose rate on the bottle.
It isn't cheap stuff though, so be prepaird to fork over about $9-$10 to treat a 20 gallon tank!!!!
