Unhappy Unique Cold Start Problem - 300 RPM

I need some help with a unique cold start problem. It’ll start right up, but everything about it is wrong…runs at about 300 RPM and sounds like it’s running on two broken cylinders (like an old worn out lawn mower engine inside the engine itself). I don’t know how else to explain it other than it sounds like a completely different engine. When it does this, there’s no response to the gas pedal and there’s nothing lit on the instrument panel to indicate something is wrong--no check engine lights. The only way to get it to cycle is to shut it off and restart it. There’s been a few times where I had to shut it off and restart it two or three times before it’ll act like a real engine. This is not the typical cold engine "dieseling". It’s completely different. Once it “truly” starts, it’ll go into its standard rumble bumble until it warms up. More often than not it’ll happen each and every morning and then be fine the rest of the day, even if it sits at work for 10-11 hours. Some mornings its completely fine...usually on the mornings when the dealer ops checks it. Funny how that works, aint it?

Other data points:
- 04/CC/4X4/6 Spd manual/Nov 03 build date/41K/stock:
- It happens predominately during the first morning start, but can happen later in the day.
- It has been cold here (around 10 degrees F), but it’ll still happen when plugged in and/or left in the garage over night (60 degrees or so).
- It’s been to the dealer 3 times with the following work completed
-- Fuel filters, oil filter, EGR valve (still plugged in BTW), EGR cooler (to repair an aggressive coolant leak), and glow plug relay.
-- Injectors, fuel pressure, electrical power, and glow plugs have been verified good each time it’s been in there
-- It’s up to date on all flashes to include the newest TSB where the injectors spool for 30 seconds after it shuts off (a smart move, I think).
-- The dealership appears to be working this issue in good faith and I like to think I have a good relationship with my service rep…both of us are always polite, patient, professional, and dedicated to the cause.
-- What else can I offer here... --
- I’m using good fuel with DK and never let it get below half full; averaging around 15.5mpg
- Primarily used as a daily driver, but my one way commute to work is about 15 miles w/mixed driving
- I regularly exercise the “drive it like you stole it” routine to help keep it clean

<O Obviously, I need to bring it back in to the dealer but they’re regularly back logged with diesel work and it usually takes them about 2-3 days after I drop it off before they get around to it (appointment or no appointment). That siad, I’d like to go back in with something to offer them...something that'll help facilitate a fix action.

Let the dealer have it for a cold morning start up, I am sure one of our resident techs will be along shortly to explain the proceedure.

All three times its been dropped of the night prior, and all three times its acted stupid in front of them during their morning start up, and all three times they seem to find "the" fix (see work list above). What I need to do is leave it there for a few extra days for them to verify a good fix action.

Sounds like theres an issue with icp pressure. Nov. 03 build.....the icp sensor should be on the pass side valve cover. Try unplugging it, then starting. That will make it default to 900 psi or so. If there is no problem then, biased icp sensor would be my guess.

No luck with the ICP unplug test. All I got was a CEL. Any other ideals?...anybody?

how about try another dealer. ask around, because if they are good, they will fix it not throw parts at it.
