Malfunction or operator error? That's a pretty odd accident for a sub.
operator error, the chart they had was not an updated one that was required to have onboard (assistant navigator fault) and also upon approval of the chart used (it goes through the chain of command for approval with all the cute markings and nav hazards) it was not noticed this navigation hazard and appropriate marking on the map were not placed as for the sub to have a buffer to go around.
also the idiot QMOW (quarter master of the watch) laid the track down (or someone did) and did not bother to thoughly check the subs track it was following, in which he had, it would have been clearly noticable.
so, in effect, stupid human error that was easily avoidable and it resulted in one death.
