1992 e-350 drum brake problem

I am about sick of the read drums on my 1992 e-350. I replaced the hole meal deal (drums, shoes, spring kits and adjusters) about 20,000 miles ago, and since I have replaced the spring kit and adjuster strap twice. I just pulled the rear wheels off on the weekend and the adjuster strap is broken again and the pieces got jambed in the shoes and have wrecked everything again - what am I doing wrong? I have triple checked that the adjusters are in the right side (adjusting tighter not looser) and that the adjusters are at least close to right before driving the van. I have never had great brakes on this van, the pedal seems to just keep going all the way to the floor with no satisfying stopping power - especially towing a trailer. Any suggestions? With 3 1/2" shoes you would think they could suck the back tires right off if I needed them to.
