I have a 2000 7.3 and I changed the fuel filter yesterday and it went very easy except for the metric nuts on the decorative plate needing a deep dish metric socket and all I had was shorties,Anyway I drained the water out first and then removed the filter and rubber seal and replaced them and it started right up and I let it run for 5 minutes to make sure there was no leaks.
Then today I took it out around town and when I got back home I was blowing diesel all over the place,So I retightened the cap on the filter and again ran it for 5 minutes and no leaks so far is it common to have to retighten the cap on the filter? wish me luck I hope it is sealed good now.
I've never had mine loosen up.... Only thing comes to mind is the orientation of the lid o-ring. Every instruction sheet says to pay close attention to that. That coud be why..
I haven't heard if it is common to have to retighten. Most people complain on how tight it was to remove which may have been a symptom along with the seal being put on backwards down. I have however personally had a problem with cover warpage. New/replacement ones can be had at a napa store for about $8.00.
Been there, done that. I don't think the cap loosens up, I think the cap o-ring doesn't get seated right and doesn't seal tight. It will hold for a little while and then start blowing fuel by.

When I first got my truck and replaced the fuel filter. Who ever changed it before had left the O-rings on the filter bowl. I had about 3 extra O-rings on there. It wasnt leaking thought but it seems to be like a something that could have very easely. Just take a look at it and see if you have the same problem.
