It only comes on occasionally.

Depending on what the code says, it may or may not help you. My first gut instinct is to pull all the sparkplugs, and see if any of them look and smell funny, like anti-freeze.
Pull and post the codes along with which of the three types it was. Codes are stored in CM, IIRC, for 50 duty cycles after the fault was first detected.
Along with radiator cap, pinched or obstructed return line from the burp tank. You can rule out the upper radiator hose as the culprit; the radiator cap has what amounts to a check valve and takes virtually no vac to open, thus allowing coolant return.
