Two days in a row I was on the freeway and my truck started sputtering and wouldn't accelerate. TONS of black smoke out the exhaust pipe. I pulled over and it would idle fine but would start sputtering if I pressed on the go pedal.
It never died on me and idled just fine. After a little while she would get up and go just fine.
Took it to the dealer and they couldn't reproduce the problem but said that the latest recall should fix it (reflash and replace exhaust pressure sensor). I was trying to avoid this but oh well.
Could this problem be caused by the EP sensor or the old flash?

Faulty EBP sensor will cause the symptoms you described. Sounds like the dealer located and fixed the problem. BTW the dealers usually always update your PCM to the latest version when they work on them for an engine related problem, this is general FMC recommend procedure.
