Engine erratically jumping rpms

Engine erratically jumping rpms
I have a problem. If i hold steady pressure on the gas pedal the engine rpm jumps all over the place. What might cause that? It feels like its "surging while driving" but it does it worse while in neutral. If i am in neutral and hold steady pressure it will jump from 2000 to 3400 to to 1200, etc. I would appreiciate it if some one could help me out.

the oil level is full. I use rotella oil and it has been changed recently. The truck isnt doing it all the time. it did it three times yesterday for about 30 minutes each time. but i drove it from 7 till 10 this morning and it hasnt done it yet. I also replaced the ICP sensor yesterday, would that maybe have something to do with it?
