Replace the whole upper control arm that includes the ball joint unless it has been previously replaced with an aftermarket one that feature removable balljoints. Moog supplies that style. Haven't seen any others.
Ball joints should be replaced when there is play in them greater than the allowable limit. No set mileage. Lower ones may wear out in as little as 35-40K.
