Do you want a body lift, or a suspension lift? A friend of mine makes his own body lifts. He custom fabricates his spacer blocks from A356 aluminum. The trick is lengthening the steel brake lines from the master cylinder to the frame, all the wires and hoses between the engine and cab, and those to the frame. Don't forget to deal with the clutch linkage and/or auto transmission linkage, and the steering shaft as well.
A suspension lift looks easier to me, just get together with a suspension shop/fabricator and work with them on what you want. Then it simply a matter of removing old factory parts and replacing them with the new parts.
