What Could Be Wrong?
I have a 2000 f550 with a 7.3/6 spd with 275k on it. When I start upshifting from 1st(not low) and throttle it above say 2100-2300 rpm it starts hesitating and smoking terriblly. It again does this in 2nd but when I get up some speed and rev in third or fourth to upshift to the next gear it has very little to no hesitation and no major smoke and revs up. Is this just mean the motor is worn out or could it be another problem. There isnt any check engine lights on.

I dont know if I am making my situation very clear but it doesnt seem to have anything to do with the transmission but with the speed and rpm related to the gear Im in and maybe the load put on the motor. The truck shifts throuth the gears but when in low, first and second and while I am accelerating in these gears and as the motor revs up over 2k or so under load,it just doesnt seem to have any power but just smokes like cazy. If I drive and shift the first few gears at under 2k rpms it doesnt smoke, or once it gets in the 30 mph or over range such as third fourth and fifth it seems to rev up to 3k or over without any smoke and decent power.

sounds like a fuel / air problem. change all filters fuel/ air filters first. If that doesn't work take it to the mechanic ( probably an injector problem.) chances are that the internal components of your engine are fine if you have maintained the truck.
