on the passenger valve cover there is a PCV valve that ran to the back of the intake !! its a complete PAIN to get to !! but if you trace the hard plastic line from the valve cover back it will run to a right angle rubber fitting on the back of the intake. this was dry rotted replaced it you have to remove the triton 5.4 cover to gain access to it !!

Thanks for the reply but I had already replaced that hose and still have the same issue

My 97 did the exact same thing took it to Autozone and got the same reading changed out all of the 02's didnt help. Check underneath the AIC there is a vacuum line that comes out of the manifold (its a rubber elbow) they go bad easily. The Ford dealer has a complete line with PCV for around $27. Should fix the problem.
