I don't consider the crash of the Concorde, which killed everyone on board, as something I'd refer to as just an "oops." Talk about callous and unfeeling...

I remember seeing video of the B-52 crash. The pilot was reported as being the hot shot of the wing and was selected to fly for an upcomming air show. The wing (or maybe the base) commander was aboard the plane. The pilot attempted a maneuver that was too agressive, too close to the ground. Then he ran out of altitude, air speed, and options all at the same time. Any pilot will tell you if you have 2 of the 3 you get to go home to the wife. Everyone on board perished. Kind of a waste when you realize the pilot was basically showing off. (If the news reports were accurate)
