First problems with my 2004

I was on a trip to east Texas last week when I had an injector go bad. The truck appeared to be driving fine however my perception for the last month was the truck was not getting its usual gas mileage of 16.75 in town. On the trip I stopped and turned the truck off and when I turned it back on it had a loud rough idle and developed a tremendious vibration at 50 to 70 mph. I took it to the Ford dealership in Sulphur Springs and the tech pulled a dianostic test and found injector #3 bad. It took him 2.5 hours to replace it. Truck is running smoothly and mileage is back to normal. (Based on the threads I have read in this forum the truck comes with the C95 injectors, the lastest generation. I checked the oil level and it was at the min level which suggested no fuel in the oil. Also I use Standyne Fuel Additive with each fillup.) Question for the group. Can the performance of an injector decrease over time (loss in fuel economy) then finally quit working or does it either work or not? I read somewhere in the threads that the injector sleeve should be replaced when replacing the injector. Is this a standard practice?
Is there anything else I should check to insure all the work and inspections were completed concerning the replacement of the injector?
Two weeks ago I had the motor for the power slide rear window go bad. The window would open slowly and would not close without some help. The L. Marshall dealership had to order the motor. It took the tech two days to replace the motor. I am told the motor has the cables attached to it as one unit and very difficult to remove and reinstall.
