If you don't want to save the cab a forklift is great, that's what I did in the past. Otherwise, I'd use two long chains bolted to the floor and run to the inside center, then use an engine puller. Set the chains up so that most of the weight is at the bottom, you'll probably need long chains so that the lift grabs near the top of the cab.
This will give you better control of the cab. I've had to move beds, cabs & large transfer tanks with an engine hoist. Works well BUT watch the balance, once it starts to tip it's hard to control.
If you can't get good balance, try bolting some extra weights on the bottom to make sure it doesn't tilt.
