I recently purchased a 1993 F150 shortbox with a 5.0 engine and 5 speed with approx 140,000 kms (84,000 miles) on it.
When I am accellerating between 70 and 100 km/h (approx 40 and 60 mp/h) there is an odd vibration that seems to shake the entire truck. If I let off the gas, it stops. This only happens when I am in 4th gear. If I stay in 3rd and shift into 4th at around 60 mp/h, there is no vibration.
Do I have a bad u-joint or is something else to blame?
its not the ujoint, I'm thinking tranny, some kind of clutchshudder but I'm sure more knowledgable peps will chime in shortly. Perhaps a fluid change with an anti-shudder additive might help.
If it were the ujoint, gear wouldn't matter.

I don't have the answer, but I have the same problem and have spent MANY hours researching it. First, identify which tranny you have. E4OD (code E on the door sticker) or AODE (code U on the door sticker) I think. The E4OD seems to have issues with electronics causing this shudder issue. The AODE (or 4R70W for easier searching) seems to have issues with the torque converter or the converter lockup clutch. On either, if you don't know the last time the fluid was changed, do it first since this might clear up the issue. I have the problem right now on a 4R70W and just bought the tranny filter. I will probably drain it this weekend, so I may have some more info for you later.
He's got the 5 speed manual, so the first thing that I would do is drain it and refill with synthetic Mercon III ATF. Check the magnetic drain plug for metal pieces. Fine shavings are OK, metal chunks are BAD.

Do you have any more info on this I can look up or tell me. I have a shudder I cant fine and maybe its the tranny(e4od).. Any help appreciated I am at a loss with my problem... Thanks Alot
