Both LMC Truck and NPD Truck Parts sell a variety of headers to fit most applications. If those won't do...Jeg's, I think, has several "weld-it-up-yourself" kind of kits that have all the pipes and brackets seperate so they can be custom welded to fit your application....I think they cost the same or maybe a little less than the preassambled headers, but then you have to pay someone to weld it all together.
