sounds obvious, but check your exhaust system for any loose hangers, etc. You may also want to fashion some sort of additional support for the exhaust system, as the factory setup is prone to vibration. Sometimes that sound comes from a loose tool box mounted in the bed and it transmits the sound through the frame and makes it a real nightmare to track down.

I have a '98 4x2 ext cab with a similar noise. Mine seems to occur after completing a slow speed turn after decelleration and then getting back on the gas. There's a strong 'thud' associated with it. At first I attributed it to the torque converter or AT not downshifting, but then I read in the Haynes repair manual about the occasional lubrication of the sliding portion driveshaft yoke. I lubrated it and have not noticed the problem again. However, I try not to recreate the conditions b/c the 'thud' was unnerving. Hope this helps...
