with the kickdown not attached do you have to manually shift into every gear or how does this work i have nver driven one with it unhooked i know for passing you drop into 2nd but do you have to up shift

No, with the kickdown not attached your truck will shift normally. You put it in drive and go. The ONLY thing a kickdown does is downshifts if you put the pedal to the floor. It has nothing to do with upshifting.
