Just about any auto parts store has a code reader. You have to careful which one you buy as they won't all work on all systems. Ford has two. one for 94 and below and one for 95 and above.
Depends on the computer system. Mine or a 93 cost about $30. It comes with a book of instructions and codes. You can also use the Check engine light and a jumper in the test plug to read the codes.
