belt not lining up

Has anyone had trouble with there drive belt running out of line with the pulleys on there 97 150 4.6 . I changed mine and it cut about 3/16" off the back side where it run off the harmonic balancer . Is there a way to realign this problem?

Is your tensioner worn?
I've seen them wear where they pivot and then the tensioner pulley doesn't sit straight and it kills the belt.

I had that happen to me on my 1990 mustang and it was the tension pulley so start with that first.
