Oil filter bypass failure
From what I have gathered so far, it's a common problem with the '94-96 powerstrokes and Ford has an updated oil filter housing for $135 or so. I went the cheap route and reassembled it as follows: The spring goes in first followed by the fiber disk and then the washer. I peened it into place all the way around instead of the original 4 places so it should stay this time. You powerstrokers might want to check it next time you change the oil, if you see an empty hole about the diameter of your little finger on the pan side next to the filter threads, you have this same problem. Without this valve most of the oil is bypassing your filter and could lead to engine problems due to dirty oil. Ford went with a c-clip retainer in '97 and this is the updated housing they sell now. I did not find any references to the older International diesels having this problem.
