Fairly common. Wherever in the pedal travel the PCM receives the maximum voltage signal will be the WOT position, and pushing the pedal down further will do nothing unless you adjust/reposition the TPS (throttle position sensor) thats mounted on the pedal assembly.

Yes.
Something else I just thought of. At your 85%-90% throttle, were you bumping the rev limiter (3400 rpm) or the speed limiter (~94 mph) if you were then nothing would happen with more throttle input and you could forget the rest of the stuff I just typed.
Most (if not all at this point) chips/programmers raise or remove the speed limiter, and some can be custom tuned to raise the rev limiter.
