No go in default OD

No go in default OD
There's a December 1991 F350 dually, 5.8, e4od where I work that has a few problems no one seems to be able to cure.
Number 1 is that it will not move unless you press the OD button to shut off over drive. In default mode, OD on, it just acts as if it was in neutral. Turn off OD and it will drive all right.
This has been into numerous shops and looked at by a guy who does nothing but auto transmissions, and no one can fix it.
Number 2, it labors to start, like the timing is advanced too far but the timing has been checked numerous times and found to be spot on.
Number 3, for a while now it needs premium fuel and will detonate like mad without it.
