How often do you have to replace the O2 sensor?

Usually a very long time, in the 90-100K range at least. Unless your engine burned oil for a time, you drove around with a bad pcv valve for a while, or blew a headgasket along the way and allowed radiator fluid to fly through the engine into the exhaust.

O2 sensors do not like oil or rad fluid whatsoever and die very quickly.

I beleive most mfgs. have a replace recomend aroud 85k, so if your at more than that and having issues, it's not too expensive to throw a new one in. You might want to use a tad(dont let any get on the sensor itself) of Anti Sieze compound on the threads when you re-install the new sensor.

Adding to what 94F150 said, make sure the anti-sieze is O2 sensor approved. Last time I replaced my O2 sensor, it came with a little plastic pouch of anti sieze.
