Hi. I have a 2003 E-350 Ext van with the V-10. I have not been able to see any mods for cat-back systems for this vehicle. So I assume a custom one would have to be made. Has anybody done this? And what improvements, if any, have you noticed. I wouldn't mind getting an additional 1 or 2 mpg. Thanks..
I don't know about MPG results, but here is a link to a SuperDuty with a Flowmaster 40 series. All he did was replace the muffler and added a chrome tip.

Yep, there's not much else that can be done. One positive thing us vanners have is the Y-pipe, it truly is a 'y' and not a T like in some of the pick-ups. But as far as cat-back systems, there is nothing made that I have been able to find. A new, high flow muffler is about the only option unless you can find a shop that does custom mandrel tube bending. A regular tube bender will restrict alot of your exhaust flow unless you go to a 3 1/2" or even a 4" pipe. Or you could use the Flowmaster with a dual 2 1/2 outlet and have a shop fabricate a system for you.
Now, if you could only find an air intake that wasn't as restrictive as the vans system. It is the difference in the hp rating I've been told, unfortunately there isn't anyone out there who's making such a thing yet. You would think with all the E-series used on C-class motorhomes that someone would develop a cold air intake, but as of yet I have not come across one. Good luck.
