Not sure how that could happen, here in Northern MN my lower heater (feet warmers) works great. But if your looking for hot air may I suggest getting a hold of mr rottndog! HeHeHeHe........
Don't mean to slam your post, mine does really work well, I just coldn't resist!
It must be because you have a 4x2! its to close to the ground!
Us 4x4's have plenty of ground clearance,my feet are so warm they sweat!
I know you can smell them all the way up there!

My 02 will get so warm by my feet it will make them hot. it doesn't take long either!
I would compare yours to some other 02's
You may have a blend door problem in the AC/heat plenum,not closing or opening ,or a vacum motor(actuator) sticking
Take it in to the dealer ,compare a few on there lot.

Now Rich... I think that extra 2.75 inches of height has gotton you into some thin air. Maybe you should try to get more oxygen into the cab. My '02 4x2 has so much heat it has to be turned down after about 15 minutes. Here in Illinois it gets cold, but I don't think we have seen temps much lower than the teens so far this year.

If not, take out the screws under the glove box door and remove the door. In front of you is all the blend doors and vacuum actuators for the vents. There are two actuators and a bunch of colored vacuum tubes.
I haven't done a lot with these, but each time you move from floor to bi-level to defrost, SOMETHING moves (at least one of the actuators moves a blend door). It's likely that one of your actuators is never moving. Check to see if the tube is hooked to it correctly. Manually move the one that isn't moving. Does air come out at the floor now?
If you're getting plenty of air at the floor but it's not hot, then I'm stumped. Hot air up top means you're running air through the heater core. Location of the downstream diverters doesn't affect the hot/cold blend door.
My '99F350 with a Powerstroke has such a hot heater I about burn my feet. I live in Michigan where temps drop pretty low. No problem for the ol' Ford.
