I would try crossing the large lugs with a screwdriver for about 20 seconds before a cold start and see if that helps. That should lead us in a direction.

The air and gpr relays work similarly. If you're manually grounding out the one terminal on the relay and the relay lights up, the most likely is you have a bad/weak connection between the relay and PCM.
what triggers the air intake heater? Could the AIH and GPR wiring be crossed?
From what I’ve read on FTE, the AIH is rarely turned on. During extended idle with some other factors if I remember correctly.
You could test it’s signal wire during initial key on (glow plug time). If they’re swapped then it should show ground at that time.

Thats good information. I can’t recall but it seems like @cleatus12r has previously explained how the PCM ground works and what can cause it to be weak.
Honestly, I’d probably check the wire and connector conditions and if nothing is found just activate the GPR ground by a momentary button inside the cab.
