Honestly they're all about the same, unless you wanna get really spendy, I'd just head to the local auto parts store and buy one there - given that it's an older truck they may need to order it in.
I had one on my '99 that my in-laws bought me, I was big into camo at the time. I think they paid $60 for it.
When I got my '03, it came with one embossed with Cabela's on it, and it was IDENTICAL to the one they paid $60 for, but was just smoked plastic instead of dipped camo. I looked it up and I think Cabela's was charging $50 or $60 for it.
When I got my '04, I bought a smoked one from the auto parts store for about $35 on sale, and it too was IDENTICAL to the others, even mounted the same way. It never came off, stayed put quite well, didn't flex at all on the highway, and lasted the 4-5 years I owned the truck.
