What do you mean the stem (stalk) won’t thread into the new switch? Have you looked up the part number to make sure it is the correct one? You will probably need a steering wheel puller but you might be able to rent one from the local parts store. The other method of leaving the nut on there about half way loose and pulling on the wheel doesn’t always work especially if the wheel has never been off. Once you have the wheel off the switch is right there. I have no idea why removing the column to replace the turn signal switch is being suggested. I think the whole process only took me about an hour. This is why a Haynes or Chilton manual is good to have. Not only for component torque specs but the same style column was used up to ‘91 so the process is the same for many years.

I chose one that fits the '91 f150 with tilt wheel, but the stem on mine straight up is too wide to screw into the new switch

Gotcha. If this ends up not working out, you can replace just the cam- the piece that is responsible for all those problems. $11 part, slightly less complicated to install.
