Depends on the Coolant/Antifreeze you are using. I use Zerex G05 and it's change interval is 5 years or 150k miles.
You said when is it necessary. So if it's dirty or won't pass a specific gravity test.

The anti-freeze is orange and has never been changed since new.
It is a 2015 model bought new in November of 2014. It has little over 45,000 miles.
I checked it a Refractometer. It was at -35 deg C
I am not sure since it has never been changed and is almost 9 years old.
It doesn't appear dirty as far as I can tell by looking in the refill reservoir.
I suppose I should take it in and let dealer decide if it should be changed.

When I bought the truck I thought I would be using it more than it has been used. But my life's activities slowed way down.
I really like the truck and am reluctant to trade it in on something smaller. I continue to take/ haul my military jeep to parades
in November. I forget about it's preventive maintenance. However I do get the oil changed. I make sure I drive it when
it goes into exhaust filter cleaning mode till it finishes.

My 2011 250 hasn't been changed since I bought it, in 2014. I haven't checked specific gravity or anything, and it is fine and green as new. I don't see the need to change it. Even when the water pump was changed in the same year, 2014, the guy who changed that , just added coolant to it.
But I don't subscribe to changing stuff because a manual or someone tells you to, especially the dealer.. I know I'll get some criticism, but that's okay. As far as I'm concerned, the coolant system is the least thing to worry over on my 250.
The anti corrosion additive in the orange coolant slowly depletes over time. This can cause pitting of the metal elements internally in the engine. That is why you change it regardless of the coolants appearance. Have the coolant changed with the new Motorcraft yellow. Small price to pay to continually protect a very expensive engine.
I used the Ford Yellow coolant when I flushed my 2013 a couple years back. Had the orange in it originally, and had no issues up to Nov 2023 when I traded the truck after the switch.
I was going to flush mine this year... poor time management and being busy equals it never happened.
Truck currently has 132k miles with original orange coolant. This fall, i dropped a few quarts from primary and added a few i had sitting there left over... I also added a bottle of the VC-12 revitalizer additive to it.
Bought a box of undiluted yellow coolant at the dealer... I'll have to see how much is left over because my son's 15 Fusion still has the orange.
The secondary was changed over, never flushed obviously, at dealer when they replaced the EGR cooler core under warranty, now expired, with the yellow.
The yellow is good for 10 years or 200k miles. Plus doesn't have to be tested which is a plus.
