I have to ask the question. If the 5100s extend longer than stock shocks and are designed for 2"-2.5" of lift, then would there be concern that the front springs can unseat when in full droop when 5100s are used with stock ride height? It is my understanding that the shocks limit the down travel. At minimum you would have greater articulation assuming the sway bar is not limiting articulation. Also aggravating the issue would be the ride height difference between 17-19 and 20+ and what does 2"-2.5" really mean... on a 17-19 or a 20+?

Yes, that would be a concern with a shock that has a long enough extended length to allow that, the Bilstein 5100 in question does not.

To further put this in perspective, the FOX 2.0 listed for the 17-23 Super Duty w/0-1.5" lift has an extended length of 22.28", a full inch longer than the 5100 (21.22"). The 5100 gives very little extended travel compared to the stock shocks, and only has approx 5/16" longer extended length than their stock replacement shock, the 4600.

Probably a silly question, but do the boots on the bilstein 5100's go up or down on the rears?

The shortest 5100's are shaft up, so boot up. Some of the longer lengths may be shaft down.

Seemed like the boot is close to the frame, thanks for the quick answer!

Yep, the boots go up on the 5100's. You should be able to read the sticker when installed.

This. Per Bilstein's website the stickers on their shocks should always be right side up and readable when installed.

Right on! Only one way for them to be installed, per Bilstein. Thanks for helping the OP, fellas!
