NumberDummy says he looked in the 1957-1968 paper catalog he has and found the number. Since that catalog went back to 1957 I assume it would have said if something older than 1964 used it.
That thread we are also talking about the special ordered 1965 2WD Custom Cab with round speedometer. Likely possible, but a sweep speedometer delete option probably wouldn't make sense since that would imply adding a round speedometer since you had to have something. The heater delete option makes sense since you could get a truck without a heater if you really didn't need or want one.
Hey brother,
My local person was able to cut these last night and is dropping them off sometime today.
They are $5.00 each ($20.00), I can take PayPal or Venmo
Message me your address and I'll put them in the mail.
Would you be able to make up two more of these in Silver?
Hey brother. My local person who cuts them for me has moved. I can send you the graphics file so you can try and have someone local to you cut it.
Let me know.

Thank you for replying. I should be able to find a local graphic shop to print them if you're willing to share the file. I'll direct message you with my email address. I appreciate your help
Hi, does anyone have the file for printing this custom cab decal? There is a local vinyl shop that will print it for me if I provide the file. Thanks, Tony
