A lot of things could cause this including internal engine damage, but it could also be caused by a leaking/stuck open injector or a leaking fuel pressure regulator.

I did lose one of the fuel pressure regulator screws when I replaced it. It don't get very good gas mileage maybe 9 or 10 mpg. And when i push on the gas pedal it put out **** ton of black smoke. And it has a hard time starting
Some are mounted right atop the air pump
Some 7.5's and 5.8's had 2 thermactors (air pumps)
They all had TAB and TAD solenoids to control them (thermactor air diverter and thermactor air bypass solenoids)
When I said better check the diverter valve (operation) is what I mean
Most likely you have a cracked vacuum line to one of the solenoids
Might want a dealer tech to take a look
Not a complicated system but there is quite a bit to it

The problems we used to see with backfires usually meant that the diverter valve was not diverting (farting)
So the truck would get air pumped downstream when it was supposed to be diverting to atmosphere (when diverting they make a fairly loud farting sound)(the air shoots out where you see the felt)
This downstream air at the wrong time is what was causing the backfires
Replaced a lot of valves and solenoids back in the day only to find the real culprit was a vacuum line
Did have a rash of bad tab and tad solenoids too right about your trucks era
