I am looking at buy a 1990 e350 ambulance with 150000 miles and the 7.3l diesel and a rebuilt transmission for 2000 dollars. the ambulance box has been taken off would I be able to tow or haul with this and would it be possible to later on put a pickup body on it if so what size cab and bed would fit
You should be able to tow or haul with it. Being an ambulance, it is assumed that the motor has many hours of idling that will show wear far exceeding the stated mileage, just an FYI.
E-series and F-series are vastly different in how they are built. I highly doubt any pickup body would fit the frame without extensive modifications. I have no clue how E-series ambulance frames are setup, but not likely that a pickup bed would be a direct swap.

TecGod is right. the frame rails are wider on the E series so the engine can sit lower (under the engine bay ceiling). That causes a whole bunch of issues with everything.
Sounds more like you want a truck. If so, you should just find an IDI truck. It's 100x cheaper than converting a van to a truck.
