I spoke to the manufacturer today and was asked to provide a picture of the back side of one front and one rear wheel. I removed the rear drivers wheel and this is what I saw:
The truck's studs were obviously poking out just a hair beyond the front face of the spacers. They will say this is the cause of my problem and they may be right. One of the reasons I went with 2"studs was because everything I read said the studs wouldn't need to be trimmed. Their instructions clearly state that the studs must not make contact with the back side of the wheel.

Yup, that's a big no no, big problem and could have caused your issue.
I wanted to give a final update on this. The guys at Motorsports Tech were great. They asked me to send them the spacers. They cleaned them up and pressed in new studs on all four. They also sent me all new nuts. At this point, I'm undecided about whether to take another shot with the spacers. Most likely, I will not, but at least I now have the option. If I chose to stay stock, the spacers are now in good shape and ready to be sold.
This is a very common situation where the vehicle studs were too long and hit the back of the wheel, causing the adapter/spacer studs to break. I've seen a lot of these pictures over the years. This is really the installers part for not verifying the stock studs don't protrude from the adapter/spacer, as most who are experienced installing these know better.
