Engine knock front pass side.

Engine knock front pass side at--the best I can tell around the timing chain cam sprocket.
New pugs, coils, and injectors. (Bosh lll)
Ok, no CODES. 1999 5.4 fuel trim is 0.0 short term both banks (after warm-up to 196 degs closed loop.) before pos 4.7 both banks. O2's are within 850 forward and 0.860 sb2, 2, and b1,2 .860. (oil was spilled down EXT pipe when they were unhooked and it is almost burned away- everyone needs a grandchild) At ID the RPMs are where they should be, rev engine, and the knock goes away. Then is very pronoced at ID and the fuel short time shows 4.7 to 4.8 at 2,000 and the other 3,000 RPMs.
Now get this, after holding RPMs at 2 grand for 30 secs and come off the gas truck will drop to 450 or 600 RPMs and try to die with pronoced knock.
Here is where I don't know as well. The long term will show 0.0 and may show 24 pos and then drop to a neg -3.5.
The truck has power and will turn over 16" 265 mud tires with just a little gas.
MAF is at .9 to 1.0 and at 3,000 goes up 3 off the top of my head. The TP says at the top 29 DEGS and at ID 19. somthing.
Sound not coming from bottom of engine or from a CAT.
Anyone got any ideals?

At 250k + miles, suspect the Cam chain tensioners to be worn allowing the chain a lot of slack and slop in movement.

Thanks, Bluegrass:

This truck only has 86,890.7 miles on it (On the Florida title too) and it is the tensioners blown out.
This truck was an estate sale truck and has a Ford remanufactured engine in it with the plastic tensioners in it. (Which tells me this engine was rebuilt after 2002)
Kind of want to know what happened to the engine that came in the truck. I know I never will know, but for one to only have this many miles and they are blown out makes one wonder what they were doing the day it left the shop.
While I am here I am going to change everything from the chains to the oil pump. The chains look good, but the guilds are toast and I am going to replace the timing cover (have a good one just setting around) as this one has ware marks from everything. Stuff like this is why I hate buying estate stuff, but sometimes it is good at the right price.

I started to pull the tensioner on the right side and it released its self like it was in a bind. Strangest thing I have seen in a while as if it was locked and never set free. Put pressure like it should on the chain and I am wondering if because it had no backing plate that caused the problem.
The chain, guilds, and all that looks very good with an almost new look. Since I am here I am going to change everything, even the oil pump.
My question is has anyone else ran into this before?
