Not necessarily what I'm using, but an observation, one of the most comfortable and ergonomic arm rests in any car I've had was my 83 Mustang. The arm rest has a built in handle that fits perfect, for me, at least. You can set it on your door where it fits for you, and you can hold the door easy when you open or close it. Fyi.

There's not a lot of room inside the cab of my 49 (and I swear its not because I'm twice the man I used to be). so I'm not sure what I'm going to do. I did find a set of armrests from a mid to early 60's Ford Falcon that I think will work out pretty good. They are on my "To Do" list... ...well, ok, They are setting on the shelf above the bench and have kind of been forgotten.
