Could be my reading comprehension has completely left me this morning but I can NOT figure out what you're asking. If your '98 has factory rear heat with or without A/C the connections to the heater core would be just under the blower case located left rear of the van. These would be the Quick Connect type and located under the floor, totally accessible from underneath just behind the rear tire.
The Quick Connect couplings up front and rear can be replaced with Dorman parts---they fit existing coolant hoses with typical worm-drive hose clamps. Dorman part number 800-403 is one type but these come in specific hose sizes---most Advanced Auto or AutoZone locations could provide the specific part for your year and engine type.
