So I'm still searching for a 2019 F250 Limited and trying to come up with a list of deal breakers. Things the dealer really can't install. I know there was a thread about adding the Hitch Prep but that is one I want factory installed.

I have a lowly Platinum ultimate and it has all the things you list except mud flaps. A limited should have everything on it. I know mudflaps are an available option when you build one so the dealer should be able to add them with no problem.
Why not place an order for exactly the build you want...
they are done taking orders for 2019's and there aren't the kind of deals on the 2020's yet. I'm will to fly to where ever I find what I want after I give my local guy the chance to dealer trade for it.

I was kind of shocked at the list of options that are not included on a Limited. My biggest hurdle is the color, I absolutely do not want black or white. First choice is Silver Spruce with red a close second.
You list your location as Detroit. For what its worth, every Diesel built for MI dealers (Where the order initially came from) will have rapid heat and the block heater. So unless you get a dealer trade unit from Ohio or Indiana, you will have those.
I know they can do a spray-in liner (by they, I mean they will send it out to be done by someone else...you may get a better deal on your own.) If it's free, OK. If not, I'd probably shop around and get better quality for the price.
WW liners are easy...they could do it, but you could do it yourself in about 5 minutes. I wouldn't let a deal fall apart over WW liners. Same for mudflaps. The other issue is whether they have them in stock or have to order. If they'll do it free, cool...but altogether the WW liners and mudflaps are less than $300 total and an easy install.
I don't know about TPMS w/camera. My trailer had a camera installed at the factory. I attached the monitor to a cupholder thing so I don't have to stick it to my windshield. I use tire minder TPMS. Works well for me. Don't know what the price comparison would be.
FX4 is what it is. My truck had it, but I wouldn't have asked for it if it wasn't there already. I took the stickers off before I left the dealer (it was a cool day but the sun was out and they just peeled off.)
Don't know about the hitch prep. I wouldn't think they would do a gear change, but I suppose money can get you anything.
I have rapid heat. It works perfectly. I would add that to a list of must-haves...but I don't know if it can be installed after the fact.
I'm pretty sure you can add the block heater. I live in DC so it really doesn't get cold enough here to be worth it to me.
Not sure if that was especially helpful, but if you need the 5th wheel I'd make sure you had the 5th wheel prep. Rapid heat works well for me. I have a 3.55 gear ratio. No issues, but I don't know what others would change to be worth it either way. If you live in a cold place, a block heater is probably a good idea, but I don't have experience with it. FX4 isn't worth it. My truck had it, but I wouldn't go out of my way for it. The rest are all easy DIY (or better options for the bed liner) unless you think your dealer will give you them for free.
- I just checked, 5th wheel hitch prep kit (2019 F350; HC3Z 5F057 B) is $695 + $100 for other 2 kits you need. Dealers want anywhere from $900 and up to install. Guy on youtube (shade tree) mechanic makes it look easy.
- TPMS is baked into dash. If you want to view trailer pressure, get an aftermarket. Those are better as they also show tire temperature, not just pressure.
- Spray in liner; search for "Line-X" in your area. I paid $1100 to get my F150 sprayed. That includes bed, all wheel wells and most importantly the gap between bed and tailgate. That turned out to be needed when I shoveled multi loads of river stones. Those small rocks would have scratched the gap area big-time if I only had factory spray liner.
- Mudflaps; look at the husky ones, look like better quality.

You can always add a block heater (many have found the Ford heating element already there and simply added the cord). Can also add the 5th wheel prep kit after the fact - can do either the Ford OEM, or aftermarket version. FX4 is shocks, skid plates, hill decent control and a sticker. You can definitely do better shocks, add skid plates and even get the sticker later if you want... but no idea on the hill decent control.
The 5th wheel prep is something I'd just have them do... especially if you are going to get the bed spray-in done at the factory. The rest are personal preference, but easy to just pick it up the way you want. I did the FX4... but will likely upgrade the shocks at some point.

I figured the Fx4 wasn't much but I don't really want to add a bunch of stuff. I'll never forget the fight I had ordering my 1996 SCLB PSD. I told them if it came in with 4x4 stickers I wouldn't take it... it seemed to just baffle them.
Afaik you can't install the block heater easily on a 6.7L.
I spent a lot of time with a parts guy and determined that the 6.7 doesn't always have one, and installation requires draining the coolant.
I believe all the international engines had the heater already.
I had the 5th wheel prep installed at the dealer I bought my truck from as a requirement. It was about $1200 total with Ford parts and they subbed the work out to a Rhino liner shop. Can't tell it's not factory.
