Question surging at start up

i have a 94' ranger supercab 4x4 with a 4.0lt engine and manual trany.
i have been having a problem with start up in that when i start the engine will surge to about 2500 and then will come down and sometimes cut out.
i can start again and hold the accelerator down just a little and it will high idle for about 15 secs and i can release the pedal and it will idle, after the first start up it will start normaly unless it has sat for maybe 6 hrs then it will suge again.also i have found that if i diconnact the battery to reset the system the next day it will start normally with no surging.
i have talked with other people and they have experienced the same thing although i cannot get any insight as to what it is.
i have pulled engine codes and get reference to the egr valve,i don't have one.
also i get a code saying banks #1 and #2 are lean,of course i am getting a cel when driving but usually when i am accelerating or going up a steep grade.
i have changed out the o2 sensors and other sensors too, cleaned out the iac and have verified that it is working.
i don't know what to do nex, except for breaking down and doing an engine diagnostic and am not prepared for that cost overun as yet , local dealer shop has a nasty habit of charging for things not done or that don't exist.

Have you cleaned the MAF( if it has one), checked fuel pressure, changed fuel filter?
Why doesn't it have and EGR valve? Who removed it?

egr was never there..have been advised that for at least 94' models only the california models had them..
maf looks good and clean, got juice to it.no way to check fuel pressure unless i could use a tire guage??,new filter.
recently got a code that refered to the fuel pump lever,i got efi.

I wouldn't recommend a tire gauge. It would spray fuel everywhere.
You can buy a fuel pressure gauge for under $50 at Auto-Zone if you don't want to take it to a shop.
I'm not sure what the fuel pump lever would be unless it is talking about the inertia switch, but it wouldn't run at all if it was tripped.

seem to have found it.
took another look at my maf , with a magnifying loop about 30 power , looks like what i thought was a clean job was not.looked like one side, towards the air filter , was caked with some gray gritty looking crap , flushed it off with throttle body spray ,checked it again and could see that the platinum filament is wrapped around some kind of white plastic and i could clearly see that it was indeed clean this time.
went on a 150 mile trip both country and interstate road and did not see a cel once, 1st time in over a year ,it would usually happen when accelerating hard and when pulling hills. truck ran fine although the real test will be in the morning for first start upto see if it is cured .
just goes to show that you need to be very sure you have cleaned what you had cleaned , that and a good jewlers loop.
thanks for all the input.
