My ‘78 F-150 has exhaust manifold leaks on both sides, and I know this for certain. My question is, when the truck is in park or neutral and revved, it sounds nice and smooth, but when the engine is under load and going down the road, I hear a “putt” noise. Would leaky exhaust manifold gaskets be more noticeable under load or am I looking at another issue?

i just resolved this on my 75 Highboy with stock manifolds on an FE360 engine.
And yes the leaks will be more pronounced under load.
remove your manifolds and have them resurfaced FLAT at a machine shop. Cost me $20 and it was worth it. Machine shop did it in 30 minutes while I waited.
buy the Remflex gaskets for your truck. Was like $35 on line.
buy new exhaust donuts. Another $15. Make sure your exhaust pipes are flanged properly and sleeved to accept the donut and match up to your manifolds.
reinstall the manifold with the Remflex gaskets and DO NOT over tighten. You can go back daily for the first week and slightly retighten.
