Kind of all three, depending on how you look at things.
Most definitely possible to be dealer installed, and probably an aftermarket product sold through dealers. Maybe even sold from the factory, but that I doubt, as this kind of accessory was almost always sourced at the dealer level rather than getting tossed in the bed on the assembly line.
But because it's a potentially dealer installed item, someone can purchase it after the fact and install it themselves. Even purchasing it from a truck accessory store other than the dealer is possible.
All of that is conjecture and assumption on my part however. Based on what I've seen over the years with other bits and parts. Don't know exactly the case with yours, but it's got a very good chance of being one of those scenarios.
