Did rust cause this?
Just had my exhaust manifold taken off, did rust cause this or did someone with a grinder cause it? Could this have been the source of my lean codes?

That was with the 3v engines not the 2v, looks like a grind mark where somebody try to take out a broken stud.

That’s a thermal crack.
Triton engines have very high exhaust temperatures. These high temperatures eventually lead to thermal fatigue cracks and thermally induced corrosion of the nodular iron manifold material.
As the hot exhaust gasses pass through the thermal cracks to the atmosphere, the material along the crack surface erodes, and you end up with a wide Chanel like the one in the picture.
