I have an 88 E-150 4.9L and love it. I have 2 issues and hope I can find the answer here.
During the hot weather the van never starts on the first try, always the 2nd try. After the start in the hot weather if I turn the A/C on the engine surges from about 400 rpm to 1200 rpm until I start to give it gas.
It is a 5 speed standard tranny, and the surge is annoying. It does stop after a few seconds of giving it gas.
Also, take the Idle Air Control valve off and clean it with carb or brake cleaner. It gets carboned up and will cause the idle to surge. The idle will change a bit when the air conditioner cycles on and off as well but should not be this severe. My truck actually stalled when the AC came on when I was in a turn and it was a dirty IAC that was the culprit. It took a couple of trys to get it squeaky clean though. I tested it by blowing through it (by mouth) to see if it was still bypassing, but make sure all the cleaner is gone first, it tastes terrible and probably is no good for you either.
I will try both items mentioned. After installing these parts does the battery have to be disconnected so the computer programs itself for the new/or cleaned parts?

disconnecting it clears the codes. however, before you do clear them, i would do a code scan to see what codes are stored, then run the vehicle for a while, and pull them again and compair the two results.
