I was going to post a diagram from my manual but since you got the bolt out you wont need that.. There is a special tool for removing them but I didn't see a pic of it in my manual.
I am not sure hot to get them out if they are stuck but am interesting in learning because I want to install new king pins and bushings soon on my 88 e150.

You mentioned clamp.. Is it one of these ball joint tools that you can get as a loaner tool at the parts store?
I have read to try to push it up instead of down.. Tighten the clamp then hit the end of the clamp a few times.. Tighten the clamp down and repeat..
Some seem to have success with a air hammer/chisels using a blunt tip punch.
As to heating. everything I read is to be careful and not overheat the I-beam part.
One post said to try not to overheat the pin itself so start with a little heat.
Another trick I read was to heat it just a little and use a grease gun and try to force some grease in the fittings. To me though, I would think that the pins are stuck in the I-beam and the grease fittings only grease the spindle.
